What is eForms?

eForms is an online form distribution and data collection application developed and used at the University of Manchester. The application has been developed and is managed by the Manchester medical school and is designed as tool to support student learning by enabling them to submit information relevant to their training. As an online form distribution application, eForms is also being used to capture data beyond medical education. These data are used to support current and potential students and their interactions with the University of Manchester.

Data captured through the eForms are not used for commercial purposes or made available to third parties. The data is submitted to eForms on a voluntary basis or through having a formal affiliation with the University of Manchester or one of its programmes.

Who owns eForms?

The application is owned and distributed by the University of Manchester. It is maintained, on behalf of the University of Manchester, by Noodlebaker ltd. You may contact eForms administrators at eformsupport@manchester.ac.uk.

The services provided are done so at no cost.

Who has reviewed eForms?

The application has been developed to support the undergraduate medical programme at the University of Manchester and has been made available across the University as an educational tool. The processing and handling of any data collected by the app is handled in accordance with the University of Manchester Data Protection policies. The application has undergone an internal test programme prior to public release.

How does eForms handle data?

Data at The University of Manchester is handled in compliance with UK GDPR and the Data Protection Act 2018. By using the app, users agree that the data they provide either by logging in or submitting voluntarily can be used to support their training and education. All data handling is compliant with the University of Manchester privacy policies.

What data is stored on the device?

When you use our mobile app, we may collect certain device and usage-related information, in addition to information you have provided yourself by completing the individual eforms.

Device information: We may collect information about the type of device and operating system you use. We do not ask for, access, or track any location-based information from your mobile device at any time while downloading or using our mobile apps or Services.

This database is erased when the app is uninstalled.

It is the responsibility of the device owner to protect their access to their device and hence the data store on it. We strongly recommend that you use some form of screen lock and storage encryption to protect unauthorised access to any of the data we store or display in the app.

What data is collected?

When you interact with eForms we may collect information about your device and interaction with the Service. We use cookies and other tracking technologies to collect some of this information.

Device information: We collect information about the device and applications you use to access emails sent through our Services, such as your IP address, your operating system, your browser ID, and other information about your system and connection.

Information collected by the form owner

The individual eform owner may use eForms to collect any data about you as they feel appropriate. This may include Personal Information or Confidential Information. In all cases, the owner is acting as Data Controller and all data collected by the Client using the Service is covered by the Clients own Privacy Policy and Data Protection Policies. We will never access this data without the explicit permission of the owner who has collected the data and then only for the purposes of resolving issues reported by the eform owner or the user. As the individual eform ownership and the use of the eForms app is predetermined by an existing relationship with the University of Manchester, the information requested or submitted will be relevant to services and training provided by the University.

All Information collected is stored securely according to Industry best practice.

Note If an internet connection is not available at the time of transmission, the data is buffered in a separate local database on the device. Once an internet connection is available again, these records are transmitted and deleted from this local database.

How will you protect my data?

All transmissions over the internet are encrypted using a standard TSL/SSL encryption protocol and is thus protected in transit. The data you send us is stored on systems that are access-controlled and data is only accessible by the owners of the individual eforms and system administrators.

What will you use the data for?

The use of the data will depend on the purpose of the individual eform. This will be described and specified by the form owner or articulated in documentation related to the specific training programmes delivered by the University. As eForms is designed to act as a tool to support the delivery of training, the data you submit will be used to enhance your experience.

Who will you share the data with?

The sharing of the data will be described and articulated by the owner of the individual eforms. The data will not be shared externally and will only be collected for the stated purpose. As eForms are supporting training delivered by the University of Manchester the data will be accessible to the appropriate and relevant academic and administrative staff.

Does eForms access third-party content?

Individual eforms may contains links to external websites, beyond the control of the University. If you click on a link, you will be directed to that site. Note that these external sites are not operated by the app provider. Therefore, we would advise you to review the Privacy Policy of these websites. We have no control over, and assume no responsibility for, the accuracy of content, the privacy policies, or practices of any third-party sites or services.
